      What do I need to provide to approve my Service Animal or ESA?

      Please fill out the required forms through your Alcove portal here. Alcove requires the following documentation, in alignment with ADA and FHA: 

      1.  ESA medical statement
      2. Vet records and up-to-date vaccines
      3. A picture of your pet
      ESA and Housemates for more information on ESA approval and current housemates.
